Case management is a FREE experience we offer to those who qualify. Our case management support provides a single point of contact for all treatment providers and families to streamline communication and provide continuity of care. All of our case managers are licensed and credentialed professionals who ensure our clients’ needs are met each and every day. This level of care gives our clients’ families peace of mind by knowing that a qualified professional is always looking after their loved ones.

Benefits

Case managers work with our patients and their family members to assess their needs (economic, housing, health, social, emotional, food, etc), maintain their independence and make sure they receive a variety of benefits including:

 

Economic

Economic Benefits
  • Free phone, TV and internet service, FP&L credits for low income individuals, home-delivered meals, food donations, and foodstamp application or re-certification.

  • Housing applications or re-certification (Section 8 or low income housing programs). Assisted living, retirement communities, nursing facilities, government-assisted housing and shared housing.

  • Transportation services for those who do not have private transportation and are unable to utilize public transportation. Transportation application or recertification: handicap, STS, Golden Pass.


Legal, Insurance & Social

Legal & Insurance Benefit
  • Legal help: Citizenship application, residence renovation, new passport, Florida IDs, insurance cards, government program benefits, consumer rights, and more.

  • Help with Medicare & Medicaid: Trained professionals offer unbiased, one-on-one help applying for state health insurance, understanding the benefits and resolving any problems. They also address issues related to supplemental insurance and long-term care insurance options.

  • Help with translation (Spanish/English) as well as understanding of correspondence and important notices (legal, health-related, etc).

  • Provide up-to-date information on the different ways clients can develop in their communities. This includes programs in churches, libraries, volunteering at community events, etc. Note: these programs are incredibly useful for helping clients feel empowered and stay busy, which effectively fights anxiety and depression.


Medical

Medical Benefits
  • Assessing a beneficiary’s medical and/or mental health needs. Coordinating appointments and appropriate care with PCPs (Primary Care Physicians), psychiatrists, counselors, group therapists, physical therapy centers and other medical providers. 

  • Free medical supplies such as walkers, canes, bath chairs and others. Over the counter medicines (OTC) application and monitoring.

  • Coordinating appropriate care with adult day care centers, home health services, abuse prevention programs, and other volunteering programs in the community.

  • Personal care services: help for individuals with functional impairments to bathe, dress, shop, walk, housekeep, and eat. Services also include general supervision, provision of emotional security, and assistance with securing health care from appropriate sources.

  • Help applying for disability benefits.
